2025 Community Fund Grant – Mid-Year Report
All agencies awarded 2025 Community Fund Grant funding are required to submit a mid-year outcomes report. Please complete and submit the online reporting form found below. This report helps us track progress, understand community impact, and ensure continued alignment with grant goals. All mid-year reports are due no later than December 19, 2025.

APH and the Dollywood Foundation have an exciting partnership that expands Dolly Parton’s Imagination Library (DPIL) program to provide young children who are blind and visually impaired with accessible books in print/braille and audio formats!

FISH of Cowlitz County enables people to get food, fill prescriptions, see a dentist, pay overdue water or PUD bills, get footwear or clothing required for a new job or fill a variety of other emergency needs. To learn more please visit their website.
